Caries treatment without a drill

A few years ago, a group of European scientists led by Professors Mayer-Lukel and Pari developed a technique that allows dentists to eliminate caries without resorting to this frightening tool. And anesthesia is also no longer necessary, since the new technology is absolutely painless. So far, the methodology of Professors Mayer-Lukel and Pari has not yet been widely implemented, but it should happen in the very near future, therefore, "Gazeta.Ru" in this material talks about what this technology is.

The method is based on the impregnation (infiltration) of tooth tissues with a special hardening solution, therefore, in professional language it was called infiltrative treatment. The product itself, manufactured by DMG, became known under the brand Icon (from the English Infiltration Concept — the concept of infiltration).

The developers of the technique report the possibility of eliminating the carious defect in just one visit to the doctor. To do this, the diseased tooth is carefully isolated in the oral cavity with a special rubber band to prevent saliva from getting on it during operation. Then the carious well is treated with an alcohol-containing solution to remove moisture. After that, a special gel containing hydrochloric acid is injected into the cavity. It etches the affected tooth tissues, while simultaneously destroying microorganisms that have invaded the tissue.

Then the space is filled with a special solution that hardens in blue light, which completely restores the integrity and appearance of the tooth.

The effectiveness of the infiltrative technique was confirmed in a randomized controlled clinical trial conducted in Germany (at the University of Aachen). Relapse of caries developed only in 4% of patients treated with this technology, although due to the peculiarities of this study, it was not possible to compare the effectiveness of the new and traditional methods - Icon and filling.

Another feature of the new method is the possibility of using it only if caries has affected the tooth no deeper than one—third of the dentin (the tissue that makes up the bulk of the tooth and is located directly under the enamel).

However, this is the main difficulty of using the method. As reported by "Newspaper.En" Elena Khaustova, head of the therapeutic department of the First Professorial Dental Clinic, the main problem is that a dentist, as a rule, cannot be absolutely sure how deep the caries has gone. It is almost impossible to determine "by eye" whether only a third of the dentin is affected or the pathological process has already advanced deeper.

Khaustova notes that X-ray examination could help in solving this issue, however, it is often associated with a number of errors and does not allow you to get a 100% guarantee in making the right decision. In addition, obtaining any X—ray image is by no means a harmless procedure associated with excessive radiation load for the patient. And the device itself is not in every dental office. In the case of a deeper and more extensive spread of caries, the effectiveness of the technique is sharply reduced, conditions are created for a relapse or recurrence of the lesion.
