Prediabetes and cancer: a dangerous correlation

Prediabetes is a condition that is an intermediate stage between normoglycemia and the development of type II diabetes mellitus. This condition is also noted in people with impaired glucose tolerance and an increase in fasting glucose levels, as well as in their combination.

In the new study, scientists conducted a meta-analysis of 16 clinical trials, which involved about 900 thousand volunteers from different parts of the world. The results showed that prediabetes in general is associated with a 15% increased risk of cancer pathology. As for the specific types of this pathology, in the presence of prediabetes, the relative risk of developing stomach / colon and rectum cancer increases by 1.55, liver – by 2.01, pancreas – by 1.19, breast – by 1.19, endometrium – by 1.6 times compared with people with normoglycemia. It is worth noting that the presence of prediabetes is not associated with the development of bronchial/lung, prostate, ovarian, kidney and bladder cancers. The results obtained were statistically significant even after adjusting for the age, ethnicity of the participants, as well as the period of their observation.

Scientists also noted that obesity is not only a risk factor for the development of type II diabetes mellitus, but also oncopathology. For this reason, they conducted a separate meta-analysis of clinical studies adjusted for the body mass index of participants, and found that after this correction, the risk of developing oncopathology in people with prediabetes remained increased by 22%.

According to scientists, the increased risk of cancer pathology in the presence of prediabetes may be associated with several mechanisms. Firstly, the presence of chronic hyperglycemia is associated with the presence of chronic oxidative stress and the accumulation of glycosylated end products, which can act as carcinogenic factors. Secondly, an increase in insulin resistance leads to an increase in insulin secretion, which, in turn, causes the growth and division of tumor cells. Thirdly, the presence of mutations in certain gene variants may determine the predisposition of individuals to the development of prediabetes and oncopathology.

The results obtained are important for the clinical practice of physicians, since the presence of a history of prediabetes in people over the age of 18 in the United States increased from 29% in 1999-2002 to 36% in 2007-2010. In a number of other countries, both developed and developing, there has also been an increase in the number of cases of prediabetes and type II diabetes mellitus.
